What if CAC is absent, undocumented, or the study is excluded?
For an eligible study, explicitly documenting no CAC can meet ACRad 44. Omitting CAC entirely is a documentation gap. Known coronary artery disease, prior bypass or coronary stent, and trauma or intraoperative CTs are exclusions; an assessment limited by image quality or clinical context may qualify for an exception.
This example assumes age 18 or older and no exclusion or exception. It illustrates the qualitative reporting pathway in ACR's 2026 specification (pages 7–8, PDF).
